BLS services typically include fundamental care such as CPR, setting broken bones, or controlling bleeding. For non-residents, who are generally charged higher rates, the average BLS fee is $732, with charges ranging from $365.42 to $1,400. Advanced life support (ALS) fees are even more variable, starting at $0 for residents and going up to $1,900, whereas non-residents face rates between $433 and $2,587. ALS services are divided into two tiers, offering more specialized care like cardiac monitoring and administering medications. The City of Chicago sets its own rates, charging residents $900 for BLS, $1,050 for lower-tier ALS, and $1,200 for higher-tier ALS, along with a $17-per-mile charge and an additional $25 for oxygen usage. Non-residents incur an extra $100 fee. Billing policies also differ significantly among agencies. While some accept insurance as full payment, others bill patients for any leftover balances and may even hire collection agencies to recover debts. One of the most notable changes occurred in 2002 when Medicare introduced a standardized rate schedule that adjusts annually based on inflation. Regardless of what individual providers charge, Medicare reimburses according to its predetermined rates, which range from $367.32 to $436.20 per transport. Since Medicare is the largest payer for ambulance services, the fees set by local governing bodies carry less weight. Only three locations in Cook County—Forest View, McCook, and Rosemont—still offer free ambulance rides to residents, although non-residents will still receive bills. Medicare's standardized rates highlight how the financial landscape has shifted over time.
